If you are considering making your first appointment with Dr. Miller, you may find the answers to these Frequently Asked Questions valuable.  You may also make an appointment to interview Dr. Miller for radio, TV, or an article.

Making a personal appointment with Dr. Miller is simple:

1.) Read enough to understand the nature of Dr. Miller’s Practice and what to expect in an appointment.

2.) Download and fill in the New Patient/Client Information Form (basic issues about what is the chief problem, other problems you want to deal with, address, phone, how long do you think you will need to be seen by Dr. Miller, etc.)

Please fill out as much of the form as you can. As you will see, it asks quite a few questions. Because this is a holistic approach to healing, the history of body, mind, emotions, and habits is essential to an accurate evaluation. Relax and participate in this little snapshot of your past.

3.) Print out and mail the application to:

4.) Call the office at 1-800-528-2737 or 1-530-478-1807 and choose a time for your first, and when possible, your second appointments.

5.) Prepare for your appointment by making a list of items you want to discuss in the session, have your time and date marked on your calendar or reminder device, and have the directions to the office location handy (which you will receive from our office staff either by email, FAX or by regular mail).

Book a Coaching appointment with Dr. Miller

1.) StudyWhat is a Life or Personal Coach? and decide if a Coaching appointment is right for you.

2.) Read, the Coaching Agreement  and sign and date it showing that you understand it and agree to the terms. If you have questions about it, please call the office at 1-800-528-2737 or 1-530-478-1807.
